This page hosts the map of all providers submitted to the Google Form. The map has different layers for physical and mental health care providers, as well as several subcategories within those. I try to update this once or twice weekly to keep things current as submissions come in. Please feel free to click on the source map and take a closer look.

Alphabetized directory of all of the providers listed on the map. The "Services" tags refer to what reviewers have chosen to share they use from a particular provider. The provider may offer additional services to those listed here. To see a provider's reviews, click on their name and it will take you to their spot in the community reviews page.
